SERVICE AGREEMENT
*** CUSTOMER HAS 3 DAY RIGHT OF REFUSAL TO CANCEL SERVICE AGREEMENT ***
The agreement is for an initial term of 12 months for monthly, bi-monthly, quarterly, semi-annual, annual and seasonal (March-October: Mosquito & Tick) services. Pest Management is limited to the targeted pests selected. Additional charges may be required for additional pests not marked or treatment of attics and/or crawlspaces. By signing this agreement, you accept all terms and conditions. At end of term, the agreement is reinstated based upon your service frequency. If at any time you desire to cancel this agreement prior to the 12-month term, a 25% buyout of all services NOT rendered will be required.
STATEMENT OF INSURANCE
The Georgia Structural Pest Control Act requires all pest companies to maintain insurance coverage. Information about this coverage is available upon request from Blitz Exterminators, LLC.
INITIAL TREATMENT
Initial Service assumes only minor infestation issues from the targeted pests. If it is determined at time of service that a more thorough clean-out is required, additional charges will be quoted to you and your agreement will automatically revert to a one-time pest service with no guarantee of controlling the targeted pests unless a new agreement is in place.
On our initial service, Blitz performs a "Cleanout" service where we will make the pests exit harborage areas, using flushing agents, to ensure pests contact residual pesticides. Insects are immune to treatments while in their egg casings and our products must be active to contact pests as they hatch. Please do not apply your own pesticides!
Initially you may see a slight increase in pest activity as the pest population is disrupted. This will cease as time passes & follow-up services are performed. For the initial and follow-up services, you may need to vacate any pets and people from the premise: your inspector and/or technician will advise.
ROUTINE TREATMENT
Application of insecticide(s) & rodenticide(s), both repellant and non-repellant, based on inspection & agreement, to exterior areas, and interior areas on an as-needed basis. We will provide routine pest control services, as per the subscription interval (monthly, bi-monthly, quarterly, etc.) to your soffits, window frames, 3 feet up & out from your foundation wall, and all trees/shrubs within 20 feet (or closer) to the foundation wall. Additionally, we will webster as necessary to eliminate spider webs and eggs no more than 20 feet above grade.
Routine Treatments are critical to maintaining the Pest Management barriers and preventing infestations of unwanted invaders. If you see more than the occasional pest, please call 478.254.4263 for a re-service.
GUARANTEE
One Time Pest Control: No guarantee or re-service without new agreement.
Seasonal Service: 30-day guarantee for marked target pests from last completed service date.
Quarterly Service: 90-day guarantee for marked target pests from last completed service date.
PAYMENTS
Auto-pay accounts will be charged on nearest business day before or after the chosen date (1st, 15th, or 28th as noted) to account for the date falling on holiday or weekend. Any failure of an ACH will incur return check charge ($50).
NO SHOW CLAUSE
Anytime an appointment is confirmed with you & our office, we are expecting you to allow us on & in your property within the scheduled time frame. If at any time you need to reschedule ON THE SAME DAY as the scheduled appointment, Blitz reserves the right to charge a NO SHOW FEE of $35.00 to your account for this missed/forgotten appointment.
ACCEPTANCE: By signing below, you are agreeing to receive Pest Management services according to the service frequency, service policy, & payment arrangements indicated above by Blitz Exterminators, LLC.
ELECTRONIC COMMUNICATION: You are agreeing to receive SMS & Email notifications regarding service notifications, discounts, communications via our office staff, etc.
ELECTRONIC PESTICIDE USE RECORDS: In accordance with state regulations, Blitz Exterminators, LLC is responsible to provide you with a record every time a pesticide product and/or pest system is applied. This record is required to be provided to the property owner, resident or custodian of the property. this record may include post-application precautionary information. PAYMENT ACKNOWLEDGEMENT: You agree to make payments per schedule selected in Payment Plan section above or at TIme-of-Service: Balances beyond 45 days may incur additional penalty.
